ADPHO (Adaptations)

Charter Community Housing will assist its tenants to live independently in the community by:

• Carrying out adaptations to a tenant's home following an assessment and recommendation by Oxfordshire County Council Social and Healthcare. Tenants requiring an Occupational Therapist assessment should either write to, or telephone, the Duty Occupational Therapist at:

• Following an OT referral, arranging for a high priority transfer to more suitable accommodation e.g. a bungalow with a shower facility. In some instances a grant can be paid to assist with removal expenses.
